To apply for Federal DentalBlue coverage, you must be enrolled in:

You can obtain an enrollment form by calling the local Blue Cross and Blue Shield Plan at the number indicated on the back of your ID card. Complete and sign the enrollment form and return it to the Plan.
Many other Blue Cross and Blue Shield Plans offer dental insurance, not Federal DentalBlue, to Service Benefit Plan members for an additional premium. If you are already enrolled and want to keep your current plan for 2009 and don't have any changes, you don't need to do anything. If you want to change your coverage for 2009 or have changes, you may be required to complete and submit specific forms.
