As a Blue Cross and Blue Shield Service Benefit Plan member, you are entitled to receive routine eye exams, frames, lenses, conventional contact lenses and laser vision correction at a savings when you use participating Davis Vision providers for your eye care needs.
To receive the discounts all you are required to do is use a participating provider and present your Blue Cross and Blue Shield Service Benefit Plan identification card at the time of service. The Davis Vision affinity program is not an insurance program, so there are no claims to file. All charges are handled directly between you and the Davis Vision participating provider.
You are never far from a Davis Vision provider. Using the FEP BlueVision network, you have access to over 27,000 providers nationwide. Use the Provider Directory to locate a provider in your area or call 1.800.551.3337. Provider locator services are available 24 hours a day, seven days a week. These discounts are not available at Wal-Mart.
