Few things in life are as precious as good vision. As a Blue Cross and Blue Shield Service Benefit Plan member, you are entitled to receive routine eye exams, frames, lenses, conventional contact lenses and laser vision correction at a savings when you use participating Davis Vision providers for your eye care needs.

Show your ID card and save

To receive the discounts all you are required to do is use a participating provider and present your Blue Cross and Blue Shield Service Benefit Plan identification card at the time of service. The Davis Vision affinity program is not an insurance program, so there are no claims to file. All charges are handled directly between you and the Davis Vision participating provider.

Participating providers

You are never far from a Davis Vision provider. Using the FEP BlueVision network, you have access to over 27,000 providers nationwide. Use the Provider Directory to locate a provider in your area or call 1.800.551.3337. Provider locator services are available 24 hours a day, seven days a week. These discounts are not available at Wal-Mart.

Savings

  • Eye examinations: 15% off the usual and customary charge or $5 off the retail cost
  • Refraction only: $20 when the examination is also covered by Medicare
  • Contact lens examination: 15% off the usual and customary charge or $10 off the retail cost
  • Conventional contact lenses: 20% off the usual and customary charge
  • Disposable/planned replacement contact lenses: 10% off the usual and customary charge
  • Non-prescription sunglasses: 20% off the usual and customary charge
  • Laser vision discount: Up to 25% off the provider's usual and customary charge or 5% off any special offer

The benefits under the non-FEHB programs are neither offered as part of nor guaranteed under the Blue Cross and Blue Shield contract with the US Office of Personnel Management (OPM). The cost of these benefits is not included in your Service Benefit Plan premium and charges for these eye examinations, frames and lenses do not count toward your out-of-pocket maximum.

If you are already enrolled and want to keep your current plan for 2009 and don't have any changes, you don't need to do anything. If you want to change your coverage for 2009 or have changes, you may be required to complete and submit specific forms.
