Our Blue Health Assessment can assess your lifestyle score and prioritize your health status based on how you answer the questions. It only takes about 20 minutes. And for 2010, we are pleased to announce a wellness incentive when you take the Blue Health Assessment.

What Is the Blue Health Assessment?

The Blue Health Assessment is an online health risk assessment that offers an easy way to learn more about your health. All it takes is 20 minutes and a computer. The Assessment guides you through a series of questions about your health and your past medical history, since it too can make a difference in your health. The Assessment also provides a means to look at your potential health risks, such as blood pressure, cholesterol levels and glucose levels.

All of the information you provide as part of the Blue Health Assessment is confidential and will not be shared with your employer.

After completing your Blue Health Assessment, you receive a personalized plan that describes the areas where your health may be able to use a little extra help and strategies on how to successfully change. Your customized health action plan will outline simple steps you can take to improve or maintain your health. In addition, you have access to additional programs that can help you achieve your health goals including:

  • Balance™ to help manage your weight through physical activity
  • Care™ for Your Back for better back health
  • Relax™ to help develop a stress management plan to deal with life's daily challenges
  • Nourish™ to help you make better food choices
  • Overcoming™ to enjoy a happier, more energetic outlook on life
  • Breathe™ for strategies on how to stop smoking with strategies to reduce cravings and resist relapse

MyBlue Wellness Incentive

We have great news for 2010! If you take your Blue Health Assessment in 2010, you will get a MyBlue Wellness Incentive certificate that waives the copayment for a routine physical examination or preventive office visit to your Preferred provider in 2010. That means for one office visit in 2010, you will not have to pay the $20 copayment for a Preferred primary care physician or the $30 copayment for a Preferred specialist under Standard Option, or the $25 copayment for a Preferred primacy care physician or $35 copayment for a Preferred specialist under Basic Option.

Understand your risks by taking our Blue Health Assessment in 2010 and learn how good it feels to be empowered to make changes for your health. Then print out the MyBlue Wellness Incentive certificate and your personal health plan to take with you when you visit Preferred physician for an annual physical.
