The Blue Health Assessment care plan will introduce you to a wealth of healthful tips and will show you how making small changes on a daily basis can make a big difference in your health. Whether you want to lose weight, quit smoking or learn some practical ways to better manage the stresses of daily life, Blue Health Assessment will provide you with the right framework to help you reach your specific health goals.
A 20 minute, on-line assessment, with guided questions, evaluates where you are with your health and then provides you with a customized action plan, based on your answers and tailored just for you.
After completing your on-line assessment, you'll find a wealth of motivating ideas and information to build your confidence in making some simple, healthy changes. Your personal action plan will guide you in making these small changes and will show you how some simple steps can make a big difference in your overall health.
