The vision network for FEP BlueVision and the Affinity program is specific to routine vision care and is different from the network for your medical plan.
It is important to know when you enroll in this Plan, benefits are available as described in the Plan's Federal brochure, but the continued participation of any one doctor hospital or other provider cannot be guaranteed. Also, the Blue Cross and Blue Shield Service Benefit Plan is prohibited from providing benefits for services performed by a debarred or suspended provider.
