Through our different care management programs, you can get ongoing support from your local Blue Cross and Blue Shield company to help improve your overall quality of life.

Case Management
If your chronic health condition prevents you from functioning at your fullest, or if you need help navigating the healthcare system, you may be eligible to receive services from a professional case manager. This case manager can help assess, plan and facilitate personal treatment options and care. Disease Management
Members living with chronic conditions (for example: diabetes, asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease or congestive heart failure and other conditions) can get help adopting effective self-care habits to help manage complex conditions at no extra cost.
